There are standard forms that need to be completed and documents that you need to give us no matter what you’re claiming for. We’ll tell you exactly what we need and you can simply download the relevant forms here: 

For a life cover claim:

• Death claim form: this needs to be completed by the claimant.
• Death claim form: this needs to be completed by the relevant doctor or other medical professional.
• In the case of accidental death, the claimant may also have to complete a police report form.
• A certified copy of the death certificate.
• A certified copy of the claimant's identity document.
• A certified copy of the life insured’s identity document.
• A bank statement or letter from the bank with a bank stamp confirming the claimant's bank account.
• If there are no beneficiaries, then the claimant will need to provide a letter of executorship.

3. Submit your claim

Send the documentation listed above to us in one of these two ways:

• Fax number 086 659 0135 or email us at mypolicy@hollard.co.za.
• You can also submit the documents via your broker or financial adviser.

4. Additional information

For a funeral cover claim:

• Funeral claim form: this needs to be completed by the claimant.
• In the case of accidental death, the claimant may also have to complete a police report form.
• A certified copy of the death certificate.
• A certified copy of the claimant's identity document.
• A certified copy of the life insured’s identity document.
• A bank statement or letter from the bank with a bank stamp confirming the claimant's bank account.

5. What happens next

Send us the forms

Complete and sign the forms then send them to us or your financial advisor along with any of the other documents that we’ve asked for. You can send them to us in two ways:
